why do people have guilt?

my friend whos 34 now, and has been married for ten years, is still feeling guilty about something that he did like 15 years ago. he basically dumped this girl that he truly loved, even though he was 18, for selfish reasons. they techinically never “dated” persay but they were both in love with eachother, they just didnt have the guts to verbalize it. so he gradually stopped calling her. he feels selfish because he never gave her a reason as to why he stopped calling. all she did was be there for him , and he basically threw her away like a piece of garbage. they were both cheated on by their firsts. my friend was paranoid that maybe she was only with him to avenge her prior betrayal, like revenge sex. were trying to figure out why he still feels guilty about it to this day. even though hes happily married and doesnt want anyone else. does anyone know?
