For the sex part- can't help you there. Only advice is when you catch yourself going in that direction with your thoughts, learn to realize it and start thinking about something else. As for asking her out: just ask if she wants to go to the movies, or the park, or anything small and safe. You will be nervous, so just keep a straight face, talk to her like you would anyone else, then when you're comfortable, ask.
