Maybe she meant who would become president if he dies or leaves office, and the VP has already died or left office previously.

If the VP leaves office, a new VP is appointed and confirmed by congress. In the event the president later leaves office, the appointed VP becomes Pres. That is how Gerald Ford became the first and only unelected president.

If Pres & VP both die or leave office at the same time, presidential succession is pretty clear. If pres and VP both die at the same time, the speaker of the house becomes pres, if he/she also dies, then the President pro-temp of the senate. If all four die, then it goes to the cabinet, Secratary of state, then Treasury, Defense, Attorney General, etc… Obviously the person assuming the office also needs to be elegible, ie. 35 years old, and natual born citizen. It would have to be a major disaster in order for it to get down to the cabinet. During a state of the union address, when all on the presidential succession list would be present, they ususally designate a cabinet member to not attend in the event of a disaster.
