It depends what trimester. In the first trimester sometimes the baby just stops developing too early. If it's any later than the first trimester, it's usually caused by the babies heart stopping. The woman starts to bleed and when that happens they usually seek medical attention and when a miscarriage has been confirmed the baby is usually removed from a pelvic exam (depending how far along she was).
a miscarriage basically means that the baby dies inside of you. it could be for a number of reasons. alcohol use, smoking, poor diet, even genetic issue's that can't be helped.
You'll get your period again and depending on how far along you are, you won't even notice, or you could have a still birth (giving birth to a dead baby).
