How to get written proof that my parents are letting me move out?

I’m 16, soon to be 17, and live in Arizona. My parents are getting a divorce and my dad is leaving the state for three months and I can’t risk leaving because I’m in college and financial aide already went through. I can’t live with my mom because she is an alcoholic and I am unable to tolerate her. My mother is known to use different things as black mail and I can easily see my moving out as being one of them. Both parents said that I can move out so the question is “How can I get written documentation stating that they both fully acknowledge that I moved out.”

Also, I need to give written consent to some of my future roommates to be able to take me to the hospital if something were to happen. Not to mention to the doctor and other such ordeals.

You cannot legally move out if your are a minor without getting permission from the court (such as emancipation). Your parents can get in trouble for letting you move out as it is their job to protect you.
