Can England trace the NHS questionnaire to me?

in school this week kids have been answering this questinaire bout life in general like eating, exercise, drugs, home, family, alcohol e.c.t. but there were some thing in the eating section I put but when I finished I was thinkin s**t what happens if they trace it to me I cud get in big trouble, I was just wondering if they could trace it o me! I only put the begining of the post code and it is private so no teachers are ment to read it, and my name isnt on it but they can trace it to the school and year so wud they try find who wrote it ?!?! im panicking please does anyone know?

It wouldn’t be worth trying to trace it even if it were possible. Unless you are confessing to a MAJOR crime.

no why would they if it is private and confidental they wouldnt go through it and for a start you didnt even put your name on it!
