It's pulled the rug OUT from under them...and it means what fogetxthexbroken says...a shock...a surprise...an unmet expectation.

If someone is standing on a rug and you yank it out from them they fall on the ground. I broke my little sisters arm that way haha.

So it just means you surprised them or that they are shocked about something.
