The Dr. may only know that your hymen is ruptured, not what caused it. You could have slipped on a bike, or whatever. Unless you have had inter course within the last week, she won't know the cause. Besides, no doctor should ever be in the business of making judgement.
And if you have been active, the doctor may be able to help you protect yourself from diseases and pregnancies.
If you were 18, the doctor would be required by law NOT to tell anyone about your medical history. At your age I'm not so sure. You could ask her not to tell.
