How do you go about getting a work permit?

I have been thinking about getting a job at a grocery store,but I am not 16.
I can work there,but only if I obtain a work permit.
How would I go about obtaining one??

You have to be 15 1/2 years of age and you get a work permit at your school. The employer has to fill it out and then you bring it back to school.
