Get them checked out by vet and I'd let the mom do what nature intended, and if she doesnt then step in and take care of them

Hello

I would say you need a deeper puppy box. I do not know what bred of dog you have but your puppy box should be deep enough to keep the puppies from climbing out. Especially 3 week old puppies.

At 3 weeks your Mom will be spending more time out of the litter box. She may lay next to it or even across the room. She does have an ear open for her babies at all times. You may not know this but she does. If your puppies are not sick, are well rounded and clean. Your mom is doing her job.

Make sure the puppies are not crying excessively, looking poor and that your Mom has plenty of milk.

You can call your Vet's office and if you have a good relationship with them, they will many times advise you on what to use to put your puppies in for a good litter boxs, if you are not sure what to make or use.

As I said not knowing your breed, this is as far as I can help.
