Well…I can’t imagine that most companies require a letter…but, I did work at a company where you had to fill out a “vacation request form” which needed approval before you could take the time off.
Include your name in the letter, your managers name, the amount of time off, cite what you believe is your alloted vacation amount at present and make sure to hand in the letter at least a month in advance.
Of course, local laws may vary…but, if the above doesn’t work, then your employer should at least be able to tell you what else needs to be included.
