sometimes with headphone connections, (not sure with xbox), theres this little plastic ring that goes into the headphone jack. if thats missing, then it won't stay in.
Thats unlikely, what it could be is a damaged wire in either end of the cord. It could be a damaged earphone, damaged cord right where it goes into the controller.
