It means when you're clear headed and thinking it through and have a plan, vs just reacting in anger, revenge is apparently better that way

Revenge is a dish best served cold. (I think that's closer to the original.)

The cold can refer not only to a cool-headed, calculated response, but the time it takes to cool off. Don't be hasty.

But in general, for the record, revenge is bad medicine.

This is how I see it...the best way to get back at someone is to wait until they forget what they did to you or until when they think the problem is solved and over and done with, then when they least expect it you get them back, but like Marbles said revenge is a bad medicine...
