Because when a guy gets sexually excited, the arteries carrying blood in his genitals enlarge while the veins carrying blood from the genital area are more constricted than in the non-aroused state.

the uneven blood flow causes the testicles to become enlarged by about 50% I think.. when the guy ejaculates, everything goes back to normal. If he doesn't, they experience temporary heaviness and discomfort...

and that's blue balls =)
