Job application

Hello
I'm currently applying for a job for frontline at Empire Theatres.
The online application has a box which says,

Position Held Responsibilities;

I've never seen a question like this before.
What do I write?

You've had jobs before and never seen this? Wow, it's about as common as anything else on a job app. What it's asking is, What was your JOB at your last job, and what were your duties? For instance, I used to work at a Dunkin Donuts, but my POSITION was Shift Manager, and my responsibilities included looking over the other employees, assisting in the day to day production of the shop, managing the register, doing cash counts, etc. Just write about what your title at your old job was, and what you did.

Reason why I've never seen anything like that is because I'd always be self employed.
