Put the card in your cardreader, find it on your my computer, rightclick and you should get up a list of different things, one of them being formate.
You should also be able to formate it from your phone, but I've never used a samsung gt-s5230 phone, so I can't tell you how to do it. You can also look HERE:

I have a samsung gt-s5230 phone and I cant put music or files on it .I have a micro sd and a micro sd card reader .I need to format the card .so when I plug the card into the pc I go to my coputer and the when I click removable disk it sayd incorrect...
