Personally, I'd say Singapore...pretty amazing place.

Second, Manhattan...for the size at least the area around Grand Central was amazing.

Dirtiest for it's size has to be San Francisco...*yuck*.

Probably Vancouver (Canada), Toronto (Canada) or Melbourne (Australia).
after all, those cities were in the top 5 in the world list of cleanest and most livable cities in the world

I think Zhuhai in China is the cleanest city I have been to.
Standing on the west bank of the estuary of the Pearl River to the South China Sea, Zhuhai is a gardenlike coastal city with 146 charming islands, thus it has got the fame of “a city with hundreds of islands”. With its amazing large green area, the city is a modern ecotype resort.
