I would go for a second opinion, take it to a tranny shop, if it is missing the 4th gear see how much it would cost to service it or how much a new/used one is and go from there wether or not its worth it.

I would check the transmission fluids first, if they are low top it up and see how it runs, or if its dirty (not bright red) then take it to a shop and get the transmission fluid replaced (they will drain it flush it out and replace it). If it still continues to act up then it could be starting to deteriorate inside (since it is a newer model of lexus it shouldnt, but you never know)
